A 73-year-old man with hematuria for 6 months. A biopsy revealed high-grade urothelial carcinoma. The patient underwent 3 cycles of neoadjuvant chemotherapy followed by transurethral resection of the bladder tumor. A Axial T2-weighted image shows an endophytic tumor extending to extravesical fat on the left side of bladder. B Axial diffusion-weighted image (b = 1000) and (C) apparent diffusion coefficient images show interrupted intermediate signal-intensity muscularis propria. D Axial contrast-enhanced image shows tumor early enhancement extends to the entire bladder wall and to extravesical fat on the left side of bladder. The final VI-RADS score was 5 for all readers. Histopathology confirmed a muscle-invasive bladder cancer
