A 76-year old patient with biochemical recurrence of PCa referred to our institution for imaging. His PSA level at the time of examination was 5.0 ng/ml. Contrast enhanced-T1w fatsat (A) and DWI (b800; B) at the pelvic level merely show a small (5 mm), oval, presacral lymph node (arrow), which was not considered suspicious. In the fused hybrid modalities PET-CT (C) and PET-MRI (D), however, the above-mentioned lymph node shows a focal, intensive [68Ga]PSMA-11 uptake, suggestive of metastatic involvement (arrow). In PET-CT (C), both ureters are visible laterally of the mentioned lymph node
