Forty-nine years-old female patient diagnosed with poorly differentiated squamous cervical carcinoma with a suspected lymph node metastasis showed on preoperative PET/MR. Postoperative pathology confirmed it as a metastatic lymph node. PET and PET/MR fusion images showed that FDG uptake in the tumor (SUVmax = 14.08 g/cm3, SUVmean = 9.35 g/cm3, TLG = 326.69 g, MTV = 34.94 cm3), and the ADC and IVIM image showed a high signal in the lymph node (ADCmean = 1.27 × 10− 3 mm2/s, ADCmin = 0.67 × 10− 3 mm2/s, Dmean = 0.97 × 10− 3 mm2/s, Dmin = 0.51 × 10− 3 mm2/s, D* = 35.92 × 10− 3 mm2/s, f = 0.17)
